A Study on the Characteristics of Disease, Self-Care Ability of Client for Home Health Care


Abstract
This study was done to assess home health care needs. it is a descriptive survey study to identify clients who need home health care services, to assess home health care needs related to their characteristics of disease, self-care ability.
The subjects of this study were 72 patients with cerebrovascular diseases, head & spinal cord injuries in neurosurgical ward of 2 general hospitals in pusan from July 1 to Sep. 30.
Data were obtained from medical records, interviews, and a questionnair. three instruments were used: chu's 'client selection criteria for home health care', 'assessment for rehabilitation nursing', 'self-care ability evaluation scale.
The data were analyzed by 'SPSS/PC'.
@ES The results were summarized as follows:
@EN 1. By using the established 'client selection criteria for home health care', the agreement level of selection among a nurse, the resaercher and a charge doctor was 89%: 69% of the patients were identified as client for home health care, 14%
were
suitable for outpatient care and only 6% needed inpatient care.
2. Characteristics of disease in identified clients for home health care were: all clients had pain & discomfortness, nutritional problem (96%), mobility problem (94%), intestinal problem (88%), skin problem (82%), insomnia (64%), urinary tract
problem (62%), skelectal problem (48%), circulatory problem (42%), sensory problem (36%), respiratory problem (30%), visual problem (26%), communication problem (12%), auditory problem (8%).
3. According to self evaluation of their self care ability, relatively low scores were found in 'sexual life', 'mobilization', 'dressing', 'elimination', 'personal hygiene', 'bed activity', 'feeding', 'language function', 'respiration'.
4. According to characteristics of disease & self care ability, home health care needs were :'frequently assessed home health care needs' were needs for pain & discomfortness, sexual life, mobility, nutrition, defecation, skin, insomnia,
voiding.
'occasionally assessed home health care needs' were needs for self care deficit (dressing, personal hygiene, bed activity), muscle tone & activity, circulation, respiration, sensory, visual problem, communication, auditory problem.
In conclsion, Cho's 'client selection criteria for home health care' was confirmed by this study as the reliable instrument (the agreement level was 89%). 69% of the inpatients were identified as clients for home health care and 92% of them had
economical problem, so we must provide them with home health care.
When we offer home health care, to begin with, we will consider their home health care needs for pain & discomfortness, sexual life, mobility, nutrition, defecation, skin, insomnia voiding.
